PERSONAL INFORMATION PRIVACY POLICY
As valued clients of GALLARDO & ASSOCIATES,
we want to inform you that we have implemented a system to ensure the
personal information is secure and that we shall only use it with our
client's consent. As defined, personal information is anything that we
may have in our possession that could be used to identify you. Whenever we obtain personal information from our clients,
we shall explain why we need it and how we intend to use or disclose it. We
will also ask for the client's consent to continue using or disclosing any
of the personal information that we had on file in the past. If the purpose
of our possessing the personal information changes for any reason, we shall
advise the client and seek a new consent.
If at any time the client wishes to review the personal
information which we posses relating to them, they may contact Paul
Gallardo, our Chief Information Officer. The role and scope of
responsibilities of the Chief Information Officer is to oversee the safe
management of all personal information collected, used or disclosed by GALLARDO
& ASSOCIATES. He may be reached at (416) 248-0909, and
will be happy to assist the clients in filing for access to the information.
Should the client disagree with any of the information
contained in our records and files, we will then ask the client to complete
and sign a correction form authorizing us to make the necessary changes. In
the event that the personal information changes without our knowledge, we
would ask the client to advise us of same, in order to revise our records
and files.
At such time that the personal information should expire
with our firm, and is no longer valid, we shall dispose of it in a secure
manner. Please be aware that we may be legally bound to retain certain or
some information for a specified time and shall dispose of it when
appropriate. Again it will be disposed of in a secure manner. Should the client have an issue or inquiry with any
aspect for our personal information policies, we invite them to contact Paul
Gallardo in order to commence a formal complaint. We assure you that
all such complaints shall be thoroughly investigated and the client shall be
provided with a written response upon the conclusion of such investigation.
It is our sincere belief that our information management protocol will
minimize the likelihood of discrepancies, but we want the client to know
that it is their right to satisfy themselves that their personal information
is being used appropriately.
